Our Commitment to Privacy: The DonorConnect website is operated by DonorConnect, the federally designated, nonprofit community service organization dedicated to the recovery and transplantation of organs and tissues for Utah, southeastern Idaho, western Wyoming, and Elko, Nevada. DonorConnect partners with Utah Lions Eye Bank, a nonprofit organization, that helps facilitate the gift of sight through cornea and eye donation. DonorConnect is committed to protecting the privacy of any personal information provided by site users, along with the security of information entrusted to the site. The process outlined below details DonorConnect’s privacy position.
Policy and Procedure
Requirements 1.3. Use of Cookies: During your visit to the DonorConnect website, a temporary “cookie” is stored on your computer’s hard drive. Cookies do not capture any personal information, however they are used for authentication, tracking and speeding up access to information you may want to view. When you log off the site, the session-based cookie is automatically cleared from your system. Most web browsers accept cookies, and your browser’s “Help” menu should provide options for the manipulation (i.e. preventing, accepting, or receiving notice) of new cookies. Please be aware the blocking of cookies may result in some site functionality being lost.
Information We Collect 2.1. Standard Web Server Traffic Pattern Information: General traffic, site usage, and length of stay information is collected and stored in log files. Log files are transaction records maintained on Web servers to record information such as: Service provider IP addresses, browser versions, referring websites, average number of pages requested, average duration of visit to the Site, and total visitor traffic. This type of information is shared externally only on an aggregated basis.
2.2. Personal Information: We do not collect personally identifiable information from you unless you provide it to us voluntarily and knowingly, as described below.
How Your Information is Used 3.1. Aggregate Information: Personal information may be used to generate aggregated data that does not allow the individual to be personally identified or contacted. Aggregated data shows visitor activity as a whole rather than on an individual basis and may be used to improve the effectiveness of the Registry and site.
3.2 Contact Forms: Personal information shared through contact forms on the site, including but not limited to First Name, Last Name, and Email Address, will be routed to the intended internal teams via email. These records are subject to DonorConnect’s internal record retention policy, no different from any other records stored internally at DonorConnect.
3.3. Service Providers: Third parties may be used to provide services such as credit card processing, website content management, and email management. In all cases, DonorConnect ensures that third-party service providers maintain the privacy of your information in accordance with this Privacy Statement.
3.4 Registry Information: Personal Information is collected when disclosed by you voluntarily for the purposes of registering your decision to donate. Such information is only collected for registration purposes on our registry sites, www.yesutah.org and www.yesidaho.org, when expressing your decision to donate. Legal Requirements 4.1. Other than as outlined above, personal information will only be released to third parties if a law, regulation, search warrant, subpoena or court order legally requires such disclosure. When we believe disclosure is appropriate, we may disclose Personally-Identifying Information in connection with efforts to investigate, prevent or take other action regarding illegal activity, suspected fraud or other wrongdoing; to protect and defend the rights, property or safety of Company, our users, our employees or others.
Security 5.1. Appropriate security measures (including encryption and other electronic and procedural safeguards) are used in order to prevent the loss, misuse, and alteration of personal information under DonorConnect’s control. However, no transmission over the Internet can be guaranteed to be completely secure. Consequently, DonorConnect and its service providers and their respective officers, directors, employees, and agents do not represent, warrant or guarantee that personal information will be protected against misuse, loss, or alterations.
5.2. We respect your privacy and are committed to protecting your personal information. We do not sell or share your personal information to third parties for monetary or other valuable considerations.
5.3. Collection of personal information for registration purposes is only possible when registering deliberately through the YesUtah and/or YesIdaho portals, www.yesutah.org and www.yesidaho.org.
Correcting/Updating/Deactivating Personal Information 6.1 Depending on your jurisdiction, you may have the right to opt-out of the "sharing" of your personal information collected through the DonorConnect website. 6.2.1 While DonorConnect does not collect personal information for registration purposes through the DonorConnect website, details on information modification for the registry site are provided here for your convenience.
Links 7.1. The DonorConnect websites may provide links to other third-party sites which may have different privacy policies. DonorConnect is not responsible for the privacy practices of such sites, except for any website owned or operated by DonorConnect. It is strongly encouraged you read carefully and fully understand the privacy policies of any website collecting personally identifiable information.
7.2. The DonorConnect website links to the YesUtah and YesIdaho websites. DonorConnect maintains responsibility for the privacy policies for the YesUtah website and YesIdaho website. We recommend reviewing these privacy policies when visiting these sites, as their details may differ from what is mentioned under the DonorConnect privacy policy.
7.3 The DonorConnect website links to the The DonorConnect Foundation website. The DonorConnect Foundation is a separate legal entity, and its privacy policy is maintained by the DonorConnect Foundation.
Notification of Changes 8.1. Any changes to the Privacy Statements for the DonorConnect websites will be posted on the respective sites so that users will be informed of what information is collected, how it is utilized, and under what circumstances (if any), it is disclosed. Information will be used in accordance with the Privacy Statement under which it was collected.
