DonorConnect takes public education—and the decision to register as an organ, eye, and tissue donor—seriously. We believe this is a personal, meaningful decision, and people deserve the information they need to make an informed choice, whether that choice is to register or not.
Our role is to help individuals and families feel informed and confident as they consider donation. We do that through community engagement and by providing clear, accurate, and motivating education—so more people can understand the donation process and, when appropriate, feel comfortable supporting it.
When education and outreach strengthens understanding and support for donation, that commitment is typically documented and communicated in one of two ways:
Our goal is to provide practical tools and consistent messaging so individuals and families can make the decision that’s right for them.
Thank you for saying "yes"!
Your online donor registration is now complete. Your decision could change the world for a person (as well as their family and friends) who is waiting for the gift of life. Organ, Tissue, and eye donors give hope and heal life.
Please print this page for your records and be sure to inform your family of your decision. When families first learn in the hospital that their loved one registered to be an organ donor, it can feel overwhelming. That’s why it’s important to share your decision now — tell your family and tell them why it matters to you!

To access your registration later, you will be asked to provide the registration ID listed below.
Below is a review of your donation registration. If you would like to modify any of the information, please use the Update My Registration button at the bottom of the page. You may update or remove your registration at any time.
By registering, you create a legal Document of Gift, called the Verification of Registry Document, under the Utah Anatomical Gift Act. If you are 18 or older, in the event of your death, your Verification of Registry Document serves as your legal authorization to donate and does not require authorization from any other person. If you are under 18, your Verification of Registry Document does not serve as legal authorization on its own; donation will require authorization from your parent, legal guardian, or other next of kin.
